How to File a Death Claim
We at Gleaner Life Insurance Society are sorry for your loss. We hope the following instructions will be helpful during this difficult time. There are two steps you may take to expedite the claim process:
1. You may complete the E-form below. This will notify the Home Office that you are starting the claim process. While this step is optional, notifying us in advance will allow us to process your claim more quickly when we receive the required signed claim forms at the Home Office.

2. Each living beneficiary is required to complete a separate "Claim for Death Benefits" form. Please return the signed form(s) to the Gleaner Home Office with an original certified death certificate that lists the cause of death.
If all designated beneficiaries are deceased, the forms listed below will need to be completed and returned to the Home Office with an original certified death certificate that lists the cause of death for each deceased beneficiary.
